quality — pointer / internal quality audit

Ready for accreditation every day, not the quarter before it's due.

IQA maps every audit checklist item to the measurable elements it evidences. Readiness updates as audits happen, so surveys stop being a scramble.

Built for MSQH, ISO and national standards, and the audit-evidenced elements of JCI and ACHS.

the problem

Your audits already hold the evidence. They just don't reach the standards.

Every day, departments run audits that say a great deal about compliance. But the results sit on paper or in separate systems, and turning them into accreditation evidence takes someone who knows which measurable element each finding belongs to. So the standards are never updated. Instead, the hospital rounds everyone up for a hospital-wide internal audit, pulling resources from care just to get a snapshot that's out of date as soon as it's done.

Operational audits are incapable of updating the standards they evidence.
The annual internal audit pulls staff from care for weeks.
Every area gets the same effort, whether it's strong or weak.
High compliance rates can hide a systemic weakness no single department sees.

Let everyday audits update your standards instantly, and save the internal audit for where the data points to weakness.

Connecting the dots

A lapse in patient identification in theatres, another in pharmacy, another on a ward: each department still reports 96% compliance. Linked to the same standard, JCI's IPSG.1 on identifying patients correctly, they reveal a systemic weakness no single audit would show. Semmel looks across departments, standards and audits to find the anomalies at the root of a problem.

Annual internal audit IQA
How standards are updated Once a year, by hand Instantly, by every operational audit
Internal audit effort Everyone rounded up, every area Focused where the data points to weakness
Cost Weeks of staff time, pulled from care Audits staff already do
Hidden weaknesses Missed behind high compliance rates Found by linking findings across departments

Annual approach

Daily operations → A gap → The annual audit → A compliance report

With IQA

Daily operations → Mapped to standards → Continuous compliance → Ready, every day

Map

Each audit checklist item is linked to the measurable elements it evidences: across MSQH, ISO and your national standards, and the practice-based elements of JCI and ACHS. The mapping is built in once, so nobody has to know which element each finding belongs to. The audits your departments already run become accreditation evidence, without extra work.

Monitor

Every audit rolls up into a compliance score for each standard. Trends show where practice is drifting, by department and by standard, so issues are fixed long before a survey.

Target

Instead of auditing every area every year, IQA points your internal audit to where the data shows weakness: standards that are drifting, and patterns that link findings across departments. A 96% compliance rate in every department can still hide a systemic gap; IQA finds it.

Prove

Findings, corrective actions and closures build a complete evidence trail as they happen. When surveyors ask, the evidence for each standard is ready in one click.

example

Hand hygiene

Instead of one annual figure, daily hand hygiene audits feed the relevant standards continuously, with trends and at-risk areas visible all year.

example

Environmental checks

Instead of quarterly rounds, daily environmental audits roll up into the measurable elements of several standards at once.

Questions quality and accreditation teams ask

Which standards does IQA support? +

MSQH, ISO and national standards, where measurable elements translate into audit checklists, and several at once. For JCI and ACHS, IQA maps the elements audits can evidence, such as patient identification, infection control and medication practice.

Does IQA cover every JCI measurable element? +

No. Elements evidenced by documents, staff interviews or leadership processes sit outside IQA today. IQA covers the elements audits can evidence, and maps each one to every standard it relates to.

Do you reproduce the standards' text? +

No. Accreditation standards such as JCI's are copyrighted. IQA follows their structure, the chapter and measurable element numbering, and describes the intent of each element in our own words, never the standard's own text.

We're not JCI accredited. Can we use the framework? +

Yes. Use its audit-evidenced elements to test your practice against JCI before you apply, or to measure it against an international benchmark.

We have our own internal standards. Can we use IQA? +

Yes. Load your own framework, and map your audits to it in the same way.

Do we need to change our audits? +

No. IQA maps the audits you already run to the standards they evidence.

Does it replace our internal audit? +

It turns your internal audit from an annual exercise into a continuous one. Your team still decides what to audit; IQA shows what it adds up to.

Can one audit count towards several standards? +

Yes. Map it once, and it evidences every standard it relates to.

Can we see readiness across several hospitals? +

Yes. Hospital groups see readiness by hospital and by standard in one view.

How is it priced? +

As an annual subscription. We'll give you an exact quote for your hospital.

Some answers depend on your hospital. We'll cover them with you.
